Website forms are intended for basic contact and scheduling information only. Please do not submit medical records, diagnoses, Social Security numbers, Medicaid identification numbers or other sensitive clinical information through public website forms.

Information we collect

When you submit a form, we may collect contact information such as your name, phone number, email address, town, referral organization, preferred tour window and marketing-attribution information such as campaign tags.

How information is used

Information submitted through the website is stored in the website’s business database and used to respond to inquiries, coordinate tour requests, support referral follow-up and measure marketing performance.

Email notifications

When website email notifications are enabled, basic contact information such as name, phone number, email address, town and inquiry type may be sent through the center’s configured email delivery provider so staff can respond promptly. Detailed care answers are intentionally excluded from notification emails. Visitors who provide an email address may also receive a confirmation message.

Custom campaign forms

The center may create unlisted forms for events, outreach or marketing campaigns. Information submitted through those forms is stored with the other website inquiries. These forms should not be used to submit medical records, diagnoses, Medicaid identification numbers or other protected health information.

Website usage analytics

We may record basic first-party website activity such as page views, clicks on call or tour buttons, activity-calendar opens and campaign tags. These usage events are used to understand which marketing efforts are useful. The website analytics feature is designed not to record diagnoses, medical records or other clinical information.
